Mood changes
The FDA found no increased risk of suicidal thoughts or behavior with GLP-1 medicines in its 2026 review. New depression, distress or unusual behavior still deserves attention. Your symptoms matter even when research does not show a medication-related increase in risk.
The evidence
In January 2026, the FDA requested removal of the suicidal thoughts and behavior warning from Saxenda, Wegovy and Zepbound after its comprehensive review found no increased risk. Current Wegovy and Zepbound labels reflect that change. Older articles may still describe the previous warning, so their publication dates matter when reading about mental health and these medicines.
That finding does not mean a person experiencing depression should be reassured without assessment. Nor does it establish GLP-1 medicines as treatments for depression or anxiety. The approved purpose of a weight loss medication is a separate question from the care needed for a mental health condition. Changes that begin after a prescription deserve review even when timing alone cannot establish a cause.
A history of depression or anxiety is not by itself a blanket exclusion from GLP-1 treatment. Tell the prescriber about current symptoms, previous crises, eating disorders and mental health medicines so suitability and support can be considered. The GLP-1 contraindications guide covers other restrictions. An individual decision still needs the full history, including whether symptoms are stable and who is providing ongoing mental health care.
What to do
If you live with depression or anxiety, that belongs in the history. It changes what gets monitored, not whether you are treated.
A shift in mood, motivation or sleep is worth mentioning at the next review rather than waiting to see whether it passes.
Very low intake, dehydration and poor sleep all affect mood independently, and all are common in the first weeks.
Thoughts of self-harm are not something to raise at a scheduled review. Contact a clinician or an urgent service the same day.
The wider picture
Contact a clinician promptly about new or worsening sadness, anxiety, loss of interest, agitation or major sleep changes. Describe when symptoms began, whether treatment changed and how daily activities are affected. A personalized weight loss plan should leave room to discuss distress about food, body image or unwanted comments about weight. If nausea or digestive symptoms are making eating difficult, report that too. Physical symptoms and emotional distress can both need attention; neither should be dismissed as a normal price of losing weight.
In physician-supervised care, ask how the prescriber will coordinate with your existing mental health clinician. Agree on a contact plan before the next appointment. Do not stop antidepressants or other psychiatric medicine without advice. For thoughts of suicide or self-harm, seek support immediately: in the United States, call or text 988. Call 911 for immediate danger or a life-threatening emergency. If possible, tell someone you trust and stay with another person while help is arranged. You do not need to work out whether medication caused the crisis before getting help.

Common questions
Current evidence, existing mental health care and the difference between a routine concern and a crisis.
I mentioned long-standing anxiety expecting to be turned away. Instead it went into the plan and got checked at every review.
Turned out I was barely eating and sleeping badly. Fixing those fixed the mood, which nobody online had suggested.
The hardest part was meals with family changing. First place that treated that as a real thing rather than a detail.
Asked directly about the headlines. Got what the evidence actually says rather than a brush-off in either direction.
