Dose schedule
Titration means changing a dose in planned steps. The starting dose, timing and maintenance options depend on the exact medicine and why it is prescribed. Follow the written prescription and contact the prescriber before making changes yourself.
Why low first
A medication name alone may not identify the right schedule. Wegovy injections and tablets contain the same ingredient but use different strengths, dosing frequencies and directions. An injection dose cannot be converted into a tablet dose by matching milligrams. Check the formulation and the condition being treated before using any schedule you find online.
For Zepbound weight management, the initial dose introduces treatment and is not an approved maintenance dose. Later maintenance choices depend on response and tolerability. Other products have their own rules: oral GLP-1 medicines do not all share the same escalation intervals or food instructions. A lower starting dose can help reduce digestive adverse effects without guaranteeing an easy adjustment.
Tell the prescriber about common side effects that persist or interfere with eating, drinking or ordinary activities. Describe severity and timing rather than simply saying a dose feels too strong. The clinician can apply the relevant label and assess whether to delay an increase, choose another permitted dose or reconsider treatment. There is no universal instruction to keep increasing or to remain indefinitely at any dose that seems comfortable.
How it runs
Long enough for the body to adapt and for the effect at that dose to be judged fairly.
Side effects, weight trend and how you are eating. The increase is a decision, not an automatic event on a calendar.
Staying at a dose for six or eight weeks is common and is not a setback. Pushing a dose that has not settled reliably makes things worse.
If the weight is coming off steadily at a middle dose, there is no reason to keep climbing. The highest dose is not the goal.
Maintenance
Maintenance means an ongoing treatment dose appropriate for the product and indication. It does not mean every intermediate strength is suitable for long-term use. Before changing the prescription, check for new medication interactions, changes in health and difficulty taking the medicine as directed. Within physician-supervised weight loss care, ask which dose is intended for maintenance, why it was chosen and when the decision will be revisited. Keep the current written instructions, and clarify any difference between those instructions and the pharmacy label.
A slowing weight loss timeline is a reason to consider the whole treatment plan, rather than automatically choose a higher dose. Likewise, reaching a personal goal does not establish that a lower dose will maintain it. If you are considering stopping medication or have already missed several doses, explain the interruption before restarting. The appropriate next dose can differ from the previous one. Tell the prescriber when the last dose was taken, why treatment paused and whether symptoms or other prescriptions have changed.

Common questions
Starting doses, maintenance choices, missed doses and symptoms that need attention.
Held at the same dose for eight weeks because it was working. No pressure to climb for the sake of it.
One increase was too much and they dropped me straight back. Two days later I was fine.
Settled well below the maximum and still losing steadily. Nobody tried to push me higher.
Every increase was a conversation rather than something that just happened on schedule.

Next step
The standard schedule is a starting point. How fast you move through it should depend on how you are actually doing.
